Cost of Completion of Medical Forms.

A Member will be reimbursed for reasonable and customary charges made by a Physician for the completion of medical forms required for the adjudication of Weekly Indemnity, Long Term Disability or Extended Health Benefit claims. A receipt must be submitted with the completed medical form.
